I came across this phrase while checking a point in the rules earlier today. Can going be both bad and rough? Surely it has to be either one or the other?

Perhaps the "and" covers cases where a unit has somehow contrived to be in both types at once?

Going can be Bad and Good but not sure about Bad and Rough!

You could be in both at the same time if a unit deeper than 1BW is sideways across the narrow gap, but I agree - I don't see a way to be entirely within both bad and rough at once.

Some elements can be 1½ BW deep (or even 2 BW deep in the case of WWg, CP, Lit & CWg). These are long and deep enough to theoretically be in Bad-Good-Rough all at the same time!
However, I suspect it’s nothing more than a glitch when translating into English that obscure and sometimes difficult to understand language known as ‘Barkerese’…

The element would be in both, but the middle part would be in Good Going. So it wouldn't be " entirely in Bad and/or Rough".
